Appointments

To schedule an appointment, please call our office at (812) 850-2129 or request an appointment through our website.

At our office, we know that your time is important. In order for us to make your new patient appointment as productive as possible, we ask that you arrive at our office 20 minutes prior to your scheduled time to make sure that all your information is in our computer and also answer any insurance related questions if we will be filing insurance for you. Our new patient forms can be downloaded and completed at home to bring to your first appointment

Returning patients need to plan on arriving 10 minutes before your scheduled appointment to allow us time to check you in, verify insurance, and update any additional information.


Cancellations

We realized unexpected circumstances arise and you cannot keep your appointment. If this occurs, we ask that you give us at least a 24 hour notice to cancel or reschedule your appointment. When a patient fails to show for a scheduled appointment or does not give a 24 hour notice to cancel their appointment, there will be a $25 cancellation fee charged.

We accept cash, check, Visa, Mastercard, Discover and American Express.

We also offer CareCredit financing to make dental treatment plans more affordable.

We are in-network providers for most major insurance companies with PPO plans, including:

If you do not see your insurance carrier listed, please call member services on the back of your insurance card to see if Dr. Stephanie Allen is listed as a provider

As a courtesy for our patients, if you provide your dental insurance in advance of your scheduled appointment, we will contact your insurance company to verify coverage and benefits. We will also file your claim after your services, but ultimately the patient is responsible for all charges. All efforts are made by our office to obtain as much information as available from your insurance company. We are unable to guarantee this service if your insurance details are not provided in advance of your scheduled appointment.

We will estimate to the best of our ability the amount you are responsible for paying. These are not guarantees that the insurance company will pay the amount left. You are responsible for paying your portion for services at the time service is rendered. Your portion would include deductibles and any non-covered procedures.
